My department lost a pager. How do we request a replacement?
Answer
It is extremely important to notify HITS as soon as a pager is determined lost, especially since many pagers regularly receive PHI. If your pager is lost, please call or chat with HITS to report it. HITS will gather the appropriate information and work with Compliance and other departments to ensure the risk of data loss is minimized. Please be prepared to provide the following information:
- The name of the individual or unit the pager is assigned to
- The Pager ID
- Is the pager a Statewide (black) or in-house (blue) pager?
- The approximate date and time the pager was lost (estimates are ok)
- The approximate location the pager was lost**
- Do you lock pages/Were there any locked pages on the pager?
- Alternate contact information for the staff member
- The primary location of the pager (e.g. C&W or NCRC B18)
**The location is important so our compliance team can properly address the potential risk to any PHI on your pager. Please provide the best guess. The more specific you can be with the location the better.
