The LMR application (https://lmr.med.umich.edu) is an internal web application accessible only while connected to the Michigan Medicine network (either on-site or via VPN).

To log in and access patient records, follow these requirements:

• Credentials: Use your Michigan Medicine (Level-2) username and password.

• Multifactor Authentication (MFA): Okta Verify is required for authentication.

  • Note: Per the institutional MFA project, Okta Verify has replaced Duo Mobile for this application.

• LDAP Authorization: Access is restricted to members of the HIM-PMR-USERS LDAP group.

  • You can verify your membership status using the Group Management Tool.

• Permission: Only staff granted explicit permission by a Health Information Management (HIM) manager will be authorized for access.
