MCard and Access Request Instructions for MM


Introduction

  • This document contains instructions for requesting MCards and/or access.
  • The MCard request process is the same for all of Michigan Medicine (hospital and medical school).
  • Access requests may be for individuals, or groups via a batch form.
  • The catalog item for MCard will reference this article, and include a link to the batch form that customers can download and use for requesting access for large groups of people.

Michigan Medicine MCard Requests and Access

The process for obtaining or replacing an MCard has been streamlined for both hospital and medical school employees and affiliates. This set of instructions applies to all of Michigan Medicine.

 

First MCard Issuance

If you are receiving your first MCard, please bring a valid government-issued photo ID (i.e., driver's license, state ID, passport) to the MCard office. All MCards must be picked up in person.

 

MCard Photos

Persons wearing sunglasses, hats, caps, bandanas, etc. will be required to remove them before their photo is taken. See Standard Practice Guide #601.13 for additional photo guidance.

Michigan Medicine MCard Office Locations

  • North Campus Research Complex (NCRC) MCard Office in Building 18, Room G018A
    •  If you have questions call: 734-764-9004.
    •  NCRC MCard office hours: Monday-Friday 9am - 12pm & 1pm - 5pm  (no appointment needed)
  • Hospital Campus: Michigan Medicine Security Key/ID Office, Med Inn Building, room C158

MCard Authorization

The cardholder's signature is required at the MCard office, attesting that the MCard photo and information is correct.

MCard or Access requests from individuals will be forwarded to the supervisor or other designated authorizer.

Costs

If payment is required:

For pickup at Med Inn: the cardholder must visit the UH cashier's office first, and bring the receipt to pick up the card. The cashier's office is located in University Hospital Room 2B221.

For pickup at NCRC: the cardholder may call the cashier's office at (734-936-6906) and the receipt will be emailed.

  • Contractor or Vendor card- $6
    • The sponsoring department may choose to pay for the card via shortcode
  • Lost MCard- $20
    • The sponsoring or employee's department may choose to pay for the card via shortcode
    • A person must turn in any previously issued MCard, or it will be considered Lost
    • Lost MCards must be reported immediately for de-activation until the card is recovered or replaced- call DPSS Dispatch at (734) 936-7890
      • Once replaced, a previously lost MCard cannot be turned in for fee reimbursement
  • Damage Beyond Wear and Tear: $20
    • Bending, cutting, and punching holes in the card that damages it beyond usability

There is no charge for

  • The first new MCard for faculty, staff, students and UM temporary employees
  • Replacing an expired MCard for faculty, staff, students and UM temporary employees
  • Replacing an MCard due to a data change (name change, department change, etc., with changes in effect in MPathways)
  • Replacing and MCard due to normal wear-and-tear from use
  • Replacing a card that was stolen
    • Stolen MCards must be reported immediately for de-activation until the card is recovered or replaced- call DPSS Dispatch at (734) 936-7890
    • A DPSS officer will be dispatched to contact you and complete a report. Provide the report number to the MCard office staff.

 

MCard Access

  • Vendors/Contractors will not be issued card reader access unless specifically requested by the sponsoring department
  • Faculty, staff, students and UM temporary employees will be issued the card reader access that is standard to their department
    • If additional access is required, list the building and room numbers, areas or card reader numbers, and obtain the approval and signature form the access authorizer of those areas. You may submit these requests in an attachment.
