After launching Legacy Medical Records (LMR) from MiChart, the Report Options page will display.  On the Report Options page, you can modify Date of Service and specify a particular Care Provider. In addition, you may select certain document types.

The specific custom document types that are created will vary depending on their availability in the patient record.

By default, the filter options are set to return no records. Please select the information you need using the filter options described below:

• Date of Service: The dates initially appear with default settings, but the Start Date and End Date can be modified.

• Care Provider: Choose All Care Providers or a select a specific provider by choosing Select Care Provider.

• • TIP: Search for providers using doctor number or by entering last name and first name. The system will autocomplete entries if they match a valid provider.

• Medical Documents: Select individual document types based on specific Departments or Services.

• Results to Include: Select specific result types.

• After selecting result filter options, click the GENERATE REPORT button to create a PDF file.
