Michigan Medicine User Profile Page (UPP)


Introduction

The Michigan Medicine User Profile Page (UPP) displays demographic and contact information for Michigan Medicine employees and affiliates. Many of the attributes originate from University of Michigan Human Resources - and some of them can be modified by the user.

You can access the Michigan Medicine Profile Page by navigating to https://lvl2.med.umich.edu/profile and logging in with your Michigan Medicine username and password.

The Michigan Medicine User Profile page provides allows users to:

  • Manage photo consent, review Personal Information, review appointment information, modify contact information, set emergency priority for contact information, and set your preferred email address for IAM related password notifications.
  • Change UMICH, Michigan Medicine passwords, or sync your UMICH and Michigan Medicine passwords via Password Settings.
  • Set up or modify Duo Settings.
  • Manage Your Just-in-Time (JIT) Elevated Privileged Access

Instructions

Personal Information

Photo displays your current MCard photo. 

  • Photo Consent Status displays the current status of your current photo consent status; Granted or Revoked.
  • Manage Photo Consent Michigan Medicine requests consent to use your MCard photo inside Michigan Medicine applications such as MiChart, MyChart (MyUofMHealth.org Patient Portal), Outlook, Teams, and Service Now. Click the Manage Photo Consent button to manage the consent of your photo.

University Long Title (listed to the right of your photo) is the name that describes the job(s) or position(s) a person has within the University of Michigan. If this information is incorrect please, contact HR at (734) 647-5538.

Personal Info Confirmed displays the last time you reviewed and confirmed your personal information on this profile page. Clicking the green button re-confirms the information on this page is correct and current.

Default Phone Number displays your selected Default phone number. If this information is incorrect, please edit your desired phone number in the Phone Numbers section.

Last Name displays your last name as it is stored in the Michigan Medicine Directory. This name can be updated in several ways:

  • For Providers, submit the Personal Data Changes eForm to HR and contact Medical Staff Services.
  • For House Officers, APTs, and SPTs, submit the Personal Data Changes eForm to HR and contact GME.
  • For Faculty and Staff, submit the Personal Data Changes eForm to HR.
  • For students, follow directions provided by the Office of the Registrar.
  • For Alumni, log into your alumni profile at the Alumni Association.

First Name displays your first name as it is stored in the Michigan Medicine Directory. This name can be updated using the same methods described under Last Name.

Uniqname is assigned to you and is also called username. If you Uniqname needs changed, please contact the ITS Campus Service Center by calling (734) 764-4357 or visiting https://its.umich.edu/help.

Preferred First Name allows you to change the name displayed in your Michigan Medicine Profile and Outlook. Leave blank or click the pencil icon to set or change this value.

UM Employment ID displays the unique number assigned to you (also known as UMID). If your Employee ID is incorrect, please contact Human Resources at (734) 647-5538 between the hours of 8:30 a.m. to 4:30 p.m.

MCARD Expiration Date displays the expiration date listed on your MCard.

Hospital Pager displays your University of Michigan Alphanumeric Pager Number. If your pager number is incorrect please contact Health Information Technology & Services (HITS): 

Licensed Full Name Provider (licensed) names are managed within Medical Staff Services (Cactus) and Graduate Medical Education (Medhub). If this information is incorrect, please contact your credentialing office.

 

Appointment Information

Department displays the department name(s) for your HR appointment(s). If this information is incorrect, please contact HR.

Department Number displays a numeric identifier that corresponds to your department(s) at Michigan Medicine. If this information is incorrect, please contact HR.

Job Title displays the name that describes someone's job or position. If this information is incorrect, please contact HR.

Manager is the assigned Supervisor for your HR appointment(s). If this information is incorrect, please contact HR.

Phone Numbers

The Phone Numbers section allows you to manage your valid business and personal phone numbers. You may also indicate your emergency contact preferences for use during Michigan Medicine emergencies and major incident situations. Only valid phone numbers (US or International) are accepted in this field.

  • Your Jabber phone number is tied to your Business phone number when set as Default
  • Your Default phone number is visible to all Michigan Medicine users and is your primary contact phone number. Your Default phone number is published in directories and systems, such as Outlook and MiChart, throughout Michigan Medicine. One phone number is required to be designated as the Default phone number.
  • Your listed Duo phone number is non-editable from this page. To update to this number, navigate to the Duo Settings tab.
  • Your listed InTune phone number is non-editable from this page.
  • Your listed MCommunity phone number is non-editable from this page. Refer to this documentation to update your MCommunity primary contact information.

Emergency Priority 

During Michigan Medicine emergencies and major incidents, these are your preferred phone numbers and associated priorities used to reach you during Michigan Medicine emergencies and major incident situations. 

Your Duo phone number is set to Emergency Priority 1. You can change this value by setting the Emergency Priority to 1 on a different phone number. If you have multiple phone numbers, you can also manually rank them as 1, 2, 3, etc.

Add/Edit Phone Number

  1. Select your Country from the drop down list.
  2. Enter a valid phone number in the Phone number field (hypens will be added automatically).
  3. Enter extension in the Ext. field (if applicable).
  4. Choose whether this number is a Business or Personal phone number.
    • Choose Business for any Michigan Medicine provided phone (office desk phone, iPhone, etc.).
  5. Select the applicable capabilities of your phone by choosing the corresponding Phone options checkboxes.
    • See below for more detail on each of the Phone options.
    • One phone number is required to be designated as the Default phone number.

Phone options When editing or adding new phone numbers, select the applicable functionality for the phone number.

Default Default is visible to all Michigan Medicine users and is your primary contact phone number. Your Default phone number is published in directories and systems throughout Michigan Medicine. One phone number is required to be designated as the Default phone number.

SMS Denotes a text-enabled device, generally used for identity validation/verification purposes. For example, this number can be used as part of the password Reset process.

Fax Denotes the device has fax capabilities.

Private Collected for specific business use (not generally available for consumption). 

Delegate / Assistant Designates someone other than yourself to answer phone calls on this device. For example, your Administrative Assistant.

Michigan Medicine Business Phone Signifies this is verified as a Michigan Medicine business phone number (office desk phone, iPhone, etc.).

 

Email Addresses

Displays your business Michigan Medicine email address and one or more personal (non-Michigan Medicine) email address. Your personal email address is generally used for identity validation/verification purposes. Click the Add email addresses link to add personal email addresses.

Preferred for IAM password communications

This email address will be used for communications from the Michigan Medicine Identity and Access Management team for IAM password communications, such as password expiration and password reset notifications.

Select your preferred email address for IAM password communications from the drop down list.

Password Settings

The Michigan Medicine User Profile Page can help you reset your UMICH, Michigan Medicine passwords, or or sync your UMICH and Michigan Medicine passwords.

  • To reset your UMICH password, click on the Reset Your UMICH Password drop-down and follow the instructions.
  • To reset your Michigan Medicine (Level-2) password, click on Reset Your Michigan Medicine (Level-2) Password drop-down and follow the instructions.
  • To sync your UMICH and Michigan Medicine passwords, click Sync to one UMICH password.
