Requesting a Michigan Medicine Account
- The Michigan Medicine Department that has determined your business need for access will be your account sponsor.
- The Sponsoring Department will submit the request for a Michigan Medicine (Level-2) Account.
- The Michigan Medicine Sponsoring Department will need:
- Legal First Name
- Legal Last Name
- Date of Birth, Gender
- Social Security Number
- External Phone (SMS-Enabled preferred)
- External E-Mail Address
- This sensitive information may be passed between the requested user and the Michigan Medicine sponsor via Dropbox or over the phone.
- The Michigan Medicine Sponsoring Department will need:
- Once the account request is processed, a “Welcome to Michigan Medicine” email will be sent to the new user at the email address provided, with the steps needed to setup your account.
- The Sponsoring Department will be responsible for oversight of your access, including timely renewal of access if there is a business need to do so.

Requesting an Account at Sparrow
If you are a Michigan Medicine or University of Michigan Health Employee partnering with a Sparrow Employee:
- Send this link to Sparrow liaison/sponsor: UM Health Caregiver Access Request via Service Now.
- What to expect: This creates a Service Now ticket for tracking access processing. Please reference the RITM or SCTASK in subsequent communications related to requests.
For those without a contact at Sparrow:
- Email isparrowexternalaccess@sparrow.org to initiate access.

- Note: If Sparrow does not have a business agreement in place, this will need to be reviewed and approved by Sparrow’s Privacy department per standard operating procedure.
- What to expect:
- A formal request form and Individual Access Agreement will be emailed back.
- Requestor completes and returns documentation.
- Sparrow team creates Service Now ticket for tracking access processing. Please reference the RITM or SCTASK in subsequent communications related to requests.

Requesting an Account at UMH-West
- Work with the UMH-West Department that has determined your business need for access, they will be your account sponsor.
- Note: Michigan Medicine and Sparrow accounts will be considered “Vendor” accounts for UMH-West.
- The UMH-West Sponsoring Department’s Manager will submit the “User Access Request” UAR Form (accessible on M-Net).
- Supervisor/Manager approval is Mandatory.
- Michigan Medicine Employees should use the “University of Michigan” company name.
- Users will be assigned application access based on predefined "security profiles" for their job code and department.
- Note: It is the manager's responsibility to request for changes to the default "security profiles" by sending a separate email to Information Security at isparrowexternalaccess@sparrow.org.
- Managers must work with their assigned business analyst to change EPIC template functionality.
